Forms and Instructions


Below are links to the three applications you will need to print out and complete in order to report your child’s birth and obtain a U.S. passport and social security number for him/her. Make sure to review the instructions for each form carefully before completing the forms. After completing all three forms you may proceed to the next heading, Application Process (see below)

Application Process


The child must accompany the parents to the Embassy. If your child is under age 16 and you are also applying for his/her passport, please be sure you read the additional requirements carefully regarding both parents’/legal guardians’ consent.

Important notice: Effective February 1, 2008, U.S. Department of State regulations extend the two parent/guardian consent requirement to all minors under the age of 16. The raising of the age requirement from 14 to 16 is intended to address the troubling issue of abducted and runaway children.

NOTE: If only one parent is a U.S. citizen, and that parent is not presently in Sweden, please contact the American Citizens Services Unit for further instructions.


In order to apply for a Report of Birth Abroad and passport, you must obtain an appointment by calling the ACS unit. For your convenience we have prepared a checklist for you.
