REPORTING THE BIRTH OF A CHILD IN SWEDEN TO U.S. CITIZENS
COMPLETING THE APPLICATION FOR A CONSULAR REPORT OF BIRTH ABROAD (DS-2029)
Complete sections 1 to 15 on the application form DS 2029 in block letters and black ink. This information is mandatory. Below you will find some tips which may prove useful when completing the form. (The form can be download at the bottom of this page)
Item 5 – Place of Birth: Please list the city where the hospital was located. This may not correspond to the place of birth listed on your child’s “Personbevis” since Swedish authorities list the name of the parish where the mother was registered at the time of the child’s birth.
Item 6 - Full Name: Please complete this item with your names as they appear on your passports. For those parents who have adopted their spouse's surname, please put your maiden/previous name within parentheses after your current name.
Item 10 - Address in United States: If you maintain a U.S. residence or address, please provide that here. You may also use your last place of residence in the U.S., or your parents’ U.S. address.
Item 11 - Evidence of U.S. Citizenship: Please complete this item with your U.S. passport number, date of issue, and place of issue as listed on your current passport. Foreign nationals may complete this item with only the country of their nationality.
Item 12 - Physical Presence: This is the actual time when the parent was physically present in the United States. This means that any travel outside the United States (3 weeks or longer), should be excluded. Maintaining a residence in the U.S. does not constitute physical presence. Please use the space on the form to list all periods in which you were physically within the borders of the United States to the best of your memory. If you were born in the United States, start with your birth date, or if you were born abroad, start with your first entry to the United States, and account for all periods up to the present. You may use a separate sheet of paper if needed.
Do not sign the form until requested by a Consular Officer at the Embassy.

The fee for the Consular Report of Birth Abroad and U.S. Passport is $150.00, or SEK 1050.00. You may pay in cash (USD or SEK) or by credit card.
