REPORTING THE BIRTH OF A CHILD IN SWEDEN TO U.S. CITIZENS

APPLYING FOR A U.S. PASSPORT IN CONNECTION TO A CONSULAR REPORT OF BIRTH ABROAD

Since U.S. law states that all U.S. citizens must enter and leave the Unties States on a U.S. passport, we recommend that you apply for a passport at the same time you register the birth of your child in Sweden.

Both Parents’/Legal Guardians’ Consent is Required for Children under age 16

The State Department strongly encourages both parents/legal guardians’ to apply in person for passports for their children. If both parents/ legal guardians are unable to accompany the child to the Embassy, the absent parent/ legal guardians must submit a copy of his/her ID, AND a signed Statement of Consent (Form DS 3053) granting permission for passport issuance for the minor child. As of November 1, 2004 this form must be notarized. The Embassy accepts forms signed before a Swedish notary public (to be found in the phone book under "notarius publicus") or before a U.S. notary public. U.S. embassies and Consulates worldwide perform this service free of charge. Please download Form DS 3053.

Important notice: Effective February 1, 2008, U.S. Department of State regulations extend the two parent/guardian consent requirement to all minors under the age of 16. The raising of the age requirement from 14 to 16 is intended to address the troubling issue of abducted and runaway children.

Photographs

Two identical passport photographs are required. The Embassy will reject any photographs that do not meet the specified requirements.

Fee

The fee for a minor passport (under age 16), is $85.00 or SEK 595.00. You may pay in cash (USD or SEK) or by credit card.

Passport Application

Please use Form DS 11, which must be downloaded from the Department of State’s website. Please disregard the instructions printed on pages 1-2 since that information pertains to passport applications when residing in the U.S. The DS 11 form must be printed on individual pages; double-sided applications are not accepted.
