REPORTING THE BIRTH OF A CHILD IN SWEDEN TO U.S. CITIZENS
CHECKLIST FOR AN APPLICATION FOR A CONSULAR REPORT OF BIRTH ABROAD, U.S. PASSPORT AND SOCIAL SECURITY NUMBER
Please print out this checklist and bring it to your appointment, signed and dated. You cannot apply for a report of birth & passport without a scheduled appointment.
Please check the appropriate box in front of each document you collect which pertains to you.
Completed “Application for Consular Report of Birth” (DS-2029). DO NOT SIGN!
Completed Form DS 11, Passport Application. DO NOT SIGN! Both parents’/legal guardians’ consent is required.
Completed Form SS-5 (Application for a Social Security Number) – signed by the U.S. citizen parent.
Child’s Swedish Birth Certificate - Personbevis
The Swedish birth certificate is a “Personbevis”, an extract from the Swedish tax authorities (Skatteverket). It must be issued in the child’s name, not in one of the parents’ names, and list the child’s full name, date and place of birth, names of Mother and Father, as well as names of legal guardians (vårdnashavare).
Please note: A Personbevis stating only the names of the legal guardians (vårdnadshavare) is NOT sufficient for our needs; the Personbevis must also include the titles Mother and Father (Moder och Fader). You must specifically request that this information regarding the child and both parents is included. Call the Swedish tax authorities at 0771-567567 to order the “Personbevis”. Parents married
An original or certified, sealed copy of the Marriage Certificate issued by the appropriate authorities in the country in which the marriage took place. Church certificates are not acceptable. Marriage certificates in languages other than Swedish or English must be translated by an authorized translator into Swedish or English. The original certificate must be presented along with any translation. Original certificate and one photocopy.
If the marriage took place in Sweden: A Swedish marriage certificate is a “Personbevis” issued in the name of one of the parents, stating the name of the spouse and is issued by the Swedish tax authorities (Skatteverket). It must state both spouses’ names, dates and places of birth, and date of marriage. This must be submitted on a separate Personbevis. Church certificates, “Vigselbevisi”, are not acceptable.
Either parent married previously
Original or certified, sealed copy of divorce decrees from all previous marriages, or death certificates. Swedish divorce decree can be obtained from the District Court (Tingsrätten) in the area where your divorce took place. Make sure to request a signed copy, stamped by a court official. Decrees in languages other than Swedish or English must be translated by an authorized translator into Swedish or English. The original decree(s) must be presented along with any translation. Original and one photocopy of each decree.
Evidence of parent’s US citizenship
The U.S. citizen parent must present his/her US passport. If the U.S. citizen parent was naturalized he/she must bring the original Certificate of Naturalization in addition to the passport. Original document and two photocopies.
Parents not married
Affidavit of Paternity and Support is required for an applicant born to an unmarried US citizen father. DO NOT SIGN!
Physical presence
The U.S. citizen parent must bring a list stating all periods of time spent in the U.S., from birth up until present date. This means that all trips outside of the U.S. (3 weeks or longer) should be excluded from the list.
Passport/Identification document for non-U.S. Citizen Parent
A non-U.S. citizen parent must bring his/her passport or other photo identification document, and one photocopy of it.
Photos
Two identical passport photographs are required. The Embassy will reject any photographs that do not meet the specified requirements.
Self-addressed envelope
Please bring a self-addressed, return envelope with postage to cover registered mail (SEK 61), to fit unfolded A4 papers. Address it to a parent.
