American Citizen Services Unit

To replace a lost or stolen passport, please follow the below instructions step by step:

  1. Report the loss of the passport to the police and obtain a copy of the police report. You must present the report at the Embassy. This applies to both lost and stolen passports.
  2. You need to present proof of identity and proof of U.S. citizenship. Proof of identity should include your photo, such as a driver's license. Proof of citizenship would be the original U.S. birth certificate (or Consular Report of Birth), Certificate of Naturalization, Certificate of Citizenship, or an old canceled U.S. passport. If you cannot submit any of these documents, please contact the ACS unit at stkacsinfo@state.gov or phone number +46 (0)8 783 5375.
  3. Two identical, recent, passport photos are required (taken within the last 6 months), which must be 2" x 2" (or 5 x 5 cm). They must be color photos, and must have a white or off-white background. The size of the head from the chin to the top of the head should not be smaller than 1" (2.5 cm) and not larger than 1 3/8" (3.5 cm). The photos should be a full front view with no head covering. No profile pictures please! Both ears should be visible. Make sure the photographer does not cut the pictures down to Swedish standard size, and do not cut them yourself. You may submit the entire card of 4, or 2, pictures, and we will cut the ones we need and return the unused ones to you.
  4. For children under age 16 there are additional requirements.
  5. Effective March 7, 2011, due to a new Embassy exchange rate, the fee for in person renewal of an adult passport will be $135.00 or SEK 945.00, and $105.00 or SEK 735.00, for children under the age of 16. You may pay in cash (USD or SEK) or by credit card.
  6. Passport applications (DS 11 and DS 64) are downloaded from the Department of State’s website. Please disregard the instructions printed on pages 1-2 since that information pertains to passport processing when residing in the U.S. Please download both Form DS 11 (PDF 89 KB) and Form DS 64 (PDF 367 KB).

Please note that according to U.S. law (26 USC 6039E) applicants for U.S. passports must provide a correct Social Security Number on the passport application. Failure to provide a Social Security Number if you have been issued one can result in a $500 fine enforced by the IRS.

If you have never been issued a Social Security Number, write zeros in the boxes. If you cannot remember your number, please contact the regional Federal Benefits Unit at the U.S. Embassy in Oslo for information regarding obtaining a replacement card.

The U.S. Embassy in Stockholm does not have access to Social Security Administration records.

To have your new passport mailed to you, you must also submit a self-addressed envelope with postage covering registered mail (72 SEK in postage).
