REPLACEMENT OF A LOST OR STOLEN PASSPORT
To replace a lost or stolen passport follow the below instructions step by step:
Report the loss of the passport to the police and obtain a copy of the police report. You must present the report at the Embassy. THIS APPLIES TO BOTH LOST AND STOLEN PASSPORTS.
You must apply for a replacement passport in person at the Embassy. Opening hours are between 9 and 11AM, Monday-Friday, except for U.S. and Swedish holidays. No appointment is necessary.
You need to present proof of identity and proof of U.S. citizenship. Proof of identity should include your photo, such as a driver's license. Proof of citizenship would be the original or certified copy of your U.S. birth certificate (or Consular Report of Birth), Certificate of Naturalization, Certificate of Citizenship, or an old cancelled U.S. passport. If you cannot submit any of these documents, please contact the ACS unit at phone number +46 (0)8 783 5375.
Two identical passport photos are required, which must be 2" x 2" (or 5 x 5 cm). They must be color photos, and must have a white or off-white background. The size of the head from the chin to the top of the head should not be smaller than 1" (2.5 cm) and not larger than 1 3/8" (3.5 cm). The photos should be a full front view with no head covering. NO PROFILE PICTURES PLEASE! Both ears should be visible. Click here for a list of photographers.
Make sure the photographer does not cut the pictures and do not cut them yourself. Please submit the entire card of 4, or 2, pictures, and we will cut the ones we need and return the unused ones to you.
There is a photo booth available at the Embassy, which may be used for emergencies only. The cost for using this photo booth is 70 SEK . NOTE: You MUST bring 70 SEK in cash to use the photo booth. Similar photo booths can be found elsewhere, but only the photo booths which print the pictures in a side-by-side format (four pictures, two on the top row and two underneath) are usable. Photo booths that print the pictures in a long strip are too narrow for our purposes.For children under age 16 there are additional requirements.
The fee for a replacement passport is $100.00 or SEK 640 for adults (16 or older), and $85.00 or SEK 544 for children (under 16). Personal checks are not accepted. You may pay in cash (USD or SEK) or by credit card.
Passport applications (DS 11 and DS 64) are downloaded from the Department of State’s website. Please disregard the instructions printed on pages 1-2 since that information pertains to passport processing when residing in the U.S. Please download both Form DS 11 and Form DS 64. The forms must be printed on individual pages; double-sided applications are not accepted.
PERSONAL APPEARANCE OF ALL MINORS
To enhance the accurate identification of passport applicants and aid in the prevention of international child abduction and trafficking, the Department of State requires the personal appearance of all minors applying for U.S. passports. This applies even if the child has previously been issued a passport, regardless of place of issuance. The personal appearance requirement for all minors is a further step towards ensuring the integrity of the passport application process. In addition, passsports renewal by mail is now restricted to applicants who hold a full validity passport which was issued when the applicant was 16 or older. Applicants who had not turned 16 at the time when the passport was issued cannot renew passports by mail, but must apply in person at the Embassy.
