Please note that applications for all passport services for U.S. citizens in Sweden are processed at the Embassy in Stockholm. You cannot send the application to the United States.

To Add Visa Pages To Your U.S. Passport

You must send this application and your passport to the U.S. Embassy in Stockholm for processing. The Embassy address is listed at the bottom of this page under the heading "Where to send the Application". The application form, DS 4085, is downloaded from the Department of State’s website. Please disregard the instructions listed on page 1 since they apply to passport processing in the U.S. Download form DS 4085 (PDF 59 KB).

Important Notice

As of February 1, 2011 if you wish to submit the application in person at the U.S. Embassy you need to set up an appointment. When on the appointment website choose the service called "Apply for a U.S. passport for the first time.

Fee

Effective March 7, 2011 due to exchange rate fluctuations, the fee to add visa pages to your passport will be $82.00 or SEK 574. This means that any payment in Swedish kronor received by the Embassy on, or after, March 7 must be in the amount of SEK 574.00.

Payment should be made by a bank check (postväxel), available from any bank, payable to the U.S. Embassy. Personal checks and postal money orders ("postanvisning") are not accepted.

Important notice: Please allow for approximately two weeks for processing time for extra pages when submitting the application by mail. Processing time may be as short as one day, if the passport is submitted in person, and picked up at the U.S. embassy. Be sure to provide your expected travel date on the form!!

Change of Name

Name changes in existing passports are no longer permitted. New passports must be issued to reflect a name change. You must send this application and your passport to the U.S. Embassy in Stockholm for processing. The Embassy address is listed at the bottom of this page under the heading "Where to send the Application".

  • If your passport was issued less than one year ago, you may receive a new passport in your new name without any charge. You must submit proof of your name change, i.e. a document from the Swedish tax authorities (Skatteverket) stating both old and new name, or a U.S. court order stating both old and new name, or an official U.S. marriage certificate in original. See the State Department website for information and application form, DS 5504 (PDF 99 KB). Please disregard the instructions listed on pages 1-2 since they apply to passport processing in the U.S.
  • Note: You must send the application and passport to the U.S. Embassy in Stockholm along with two new photos, taken in accordance with Department of State regulations, and a self-addressed return envelope with postage to cover the cost of registered mail (SEK 72 in postage).
  • If your passport was issued more than one year ago, you must apply for a new passport using the appropriate renewal passport application and pay the passport fee. You must submit proof of your name change, i.e. a document from the Swedish tax authorities (Skatteverket) stating both old and new name, or an official U.S. court order, or a U.S.marriage certificate in original. Please note that you can apply for a new passport in your new name by mail if you are eligible for mail-in renewal. Disregard instructions printed on pages 1-2 of the DS 82 application form as special rules apply when processing a passport at the Embassy. Correction of Data

Please contact the American Citizen Services Unit for information and forms at phone number 08-783 5375, Monday, Tuesday and Thursday between 1 and 2PM, or by email: stkacsinfo@state.gov. Please provide detailed information regarding the incorrect data in the passport, such as misspelling of name, etc.

Where to send the Application

Please send your passport, the application form, name change document if applicable, and a self-addressed return envelope with postage covering the cost for registered mail (72 SEK in postage) to the following address:

Since passports are such valuable documents, we recommend that you use registered mail (rekommenderad post) when sending a passport application to the U.S. Embassy.
